The role holder will be the Local Authority representative for the majority of CHC assessments working closely with the local (ICB) completing Checklists and Decision Support Tool assessments with a nurse assessor. The role holder will have experience and understanding of the Continuing Health Care process and National Framework.
They will be also experienced in care and support planning, as this is required for any individual who requires Local Authority led care and support if not eligible for Continuing Health Care.
