Hospital Social Worker

Sirona Medical are recruiting for a Hospital Social Worker

This role is based in our Home First Hub in King’s Lynn, West Norfolk supporting discharges from an acute hospital setting and out of County hospitals for clients resident in West Norfolk catchment area under the Discharge to Assess pathway.

Candidates are required to be onsite for this role, at St James Health Centre, West Norfolk, King’s Lynn.
Hybrid, 3 days per week in the office minimum

Skills, Knowledge and Experience

  • A qualification in social work i.e.

    Dipsw, MSc, CQSW, and CSS.
  • Registration with the HCPC as a social worker.
  • Substantial experience as a qualified social worker.
  • Knowledge of Adult Social Care
  • Hospital experience essential
