We are recruiting for a Physiotherapist to support assessments in discharge to assess (D2A) beds, clients that have left hospital following an acute episode and are unable to return home and require a period of assessment in a care home setting.



The Community Independence Service (CIS) Therapists will take referrals directly from the hospital for those clients who are discharged from hospital into a residential/nursing home while they await a package of care or require further (therapy) assessment.


These cases are prioritised to ensure that a timely assessment is completed, reducing the risk of functional deterioration.
The CIS Therapist will complete a full functional assessment of the clients current ability.

Where appropriate they will write a rehab program that can be implemented by the CIS team for the duration of the clients admission to promote maximum independence.

If appropriate, the CIS team will follow the client home once discharged and complete the rehab in the home environment.

• A degree in Physiotherapy
• HCPC Registered
• Be eligible to work within the UK
• Recent NHS D2A/Community Experience Required
