Sirona Medical's Social Work department are recuriting for a Social Worker to work with people being discharged from general hospitals within West Northants and on occasions hospitals bordering the region.

Social Worker will work alongside hospital wards ensuring appropriate measures are in place for where they are being discharged to, whether that’s their own homes, care homes or reablement services.
 
You will be reuired to undertake safeguarding enquires, undertake care act assessments, and undertake reviews.

This is a community based role visiting people in their own environment/care homes.

To be considered for this role, applicants must have the below:
Relevant Social Work qualification (BA/MA Social Work/DipSW or equivalent)
Social Work England Registered
Eligible to work within the UK
Recent frontline experience within Social Care
