Community Diabetes Nurse Specialist
An outstanding new job opportunity has arisen for a dedicated Community Diabetes Nurse Specialist to work in an exceptional hospital based in the Fleet, Hampshire area.
You will be working for one of UK's leading health care providers
You will act at intermediate level between primary care and secondary care specialist teams to enhance communication links between services to ensure best possible outcome for patient with diabetes

*To be considered for this position you must be qualified as a Registered General Nurse (RGN) with an active NMC Pin

As the Community Diabetes Nurse Specialist you key duties include:
- Promote the health and wellbeing of people with diabetes
- Provide a link between GP and consultant led specialist teams
- Provide education and support to people with diabetes and those who care for them
- Be responsible for managing a defined caseload of patients, using theoretical knowledge and high levels of practical experience to formulate a specialised programme of care
- Accept referrals from anyone involved in caring for people with diabetes and from patients themselves
- Provide education and support to Health Care Professionals
The following skills and experience would be preferred and beneficial for the role:
- Evidence of managing people with diabetes and completion of a qualification in diabetes is desirable
- You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e.
Outlook, Word, Excel, Teams - Passionate for specialist nursing
- Excellent communication skills
- Good organisational skills
- Worked within a community nursing team
The successful Nurse will receive an excellent salary up to £39,027 per annum.
This exciting position is a Full Time role working Mon-Fri from 9am to 5pm.
