Sirona Medical's Social Work Department are recruiting for a Mental Health Inpatient Link worker Role (working age adults) The role is to establish good practices in relation to timely discharge and link into a Virtual discharge hub.

This will include promoting and safeguarding the rights of patients, ensuring capacity considerations are embedded throughout the admission.

Fulfilling the Councils responsibilities under the Care Act 2014, including Prevention and conducting Safeguarding Enquiries.

Providing a social perspective to inpatient assessment, enabling positive risk taking in relation to discharge with a discharge to assess approach.

Promoting a home first and community focus of admission and discharge, educating staff on this approach, process, and perspective in relation to the social care responsibilities.

Supporting the virtual discharge hub in understanding the roles and function of social care, and supporting efficient referral pathways, support planning and funding requests.

Participate and ensure the Delayed Transfers of care (DTOCs) procedure is implemented correctly in a fair and objective manner.

Ensuring the implementation of s117 policy and clear consideration of aftercare needs are recorded.

Linking in with Social Care colleagues to feedback on patient progress and any barriers to discharge.

Provide feedback to senior colleagues on matters in relation to flow and barriers to discharge.

• A relevant Social Work qualification (BA/MA Social Work/DipSW or equivalent)
• Be Social Work England Registered
• Be eligible to work within the UK
• Be a car driver and have access to a vehicle 
• Enhanced DBS
