Social Worker Reablement and Hospital Discharge

Social Worker Reablement and Hospital Discharge Team 

Duties:

  • Attend board rounds on the wards
  • Complete ASC assessments
  • Interventions to maximise independence and minimise dependency on long term services
  • Duty presence for the acute trust
  • Completed Mental Capacity Assessments and Best Interests Decisions
  • Attend Discharge Planning Meetings/Professional meetings
  • Work in partnership with other health and social care colleagues
  • Ensure safely and timely discharges from hospital
 
To apply for this role you must have:
  • A relevant Social Work qualification (BA/MA Social Work/DipSW or equivalent)
  • Social Work England Registration 
  • Be eligible to work within the UK
  • Full UK drivers license and have access to a vehicle 
  • Experience of working within a hospital/Reablement setting
